I have made a NEW Roster/Waiver form for you to use. It still has the SAME exact Information as all those in the past. This one is better because

1. I've made it so you can TYPE the Team Name, City & State, your Team Sanction #

2.I've made it so you can TYPE the Players Names & Date of birth MM/DD/YYYY (Example  05/03/2000)

3.Also if you know who will be signing as Parent / Guardian you can type that as well.

This WILL fit on ONE page no matter what text size you are viewing!!
